3. payroll records of past 12 months 過(guò)去12個(gè)月的工資表
4. piece rate records of past 12 months (if applicable)計(jì)件工人過(guò)去12個(gè)月的計(jì)件記錄（如適用）
5. attendance records of past 12 months過(guò)去12個(gè)月的考勤記錄
6. official overtime waiver/ comprehensive working hour system permit (if applicable)當(dāng)?shù)貏趧?dòng)局批發(fā)的加班批文或綜合工時(shí)批文（如適用）
